All-on-4 dental implants
All-on-4 dental implants are designed for patients lacking all or most of their teeth, and are used in cases of bone deficiency. This method makes it possible to restore the function of the jaw system with a relatively low investment of time and money.
With this method, the requirements for the condition of the jawbone are significantly less than other methods. In All-on-4 implants, the two outer artificial roots are implanted at an angle of 30-45° and the two middle roots are implanted perpendicular to the dentition.
Therefore
there is no need to involve the maxillary sinuses, which would often preclude an upper jaw augmentation surgery
a competently planned and lab-processed design enables an even distribution of the load on the inserted implants

Reasons we prescribe the All-on-4 implants
All-on-4 implants are a modern method that in most cases does not require any additional surgery.
Dental restoration with this technology shall be as indicated in the following cases:
Total adentia

Total adentia is a complete absence of teeth; typically, parts of the jawbone are atrophied, and have lost their original volume.
Partial adentia

Partial adentia is a loss of a significant number of teeth; the remaining teeth are loose and cannot be used as a support for a normal bridge.
Periodontitis

Periodontitis is an inflammation of the periodontal tissues and weakening of a ligamentous apparatus that may lead to tooth loss, and bone degeneration. There is not enough bone to place a large number of implants.
Advantages of the method

During treatment, the doctor treats areas with minimal bone resorption, as well as deep layers. This allows dental implants to be inserted without the need for a bone graft. If the upper jawbone has no volume and the maxillary sinuses are extremely close, the All-on-4 will be effective.
For temporization, we use acrylic overdenture restorations. The new teeth and artificial gum look as natural as possible.
The patient will be able to chew immediately after the treatment with the new teeth, habituating to new teeth and gradually increasing the load. Within 21 days, the chewing function could be completely restored.
The surgery is carried out using a prefabricated surgical template. This way, the implants are implanted in the bone tissue at the correct angle with minimum trauma.
The disadvantages of the All-on-4 are the increased load on the jaw and a possible decrease in the lifespan of the implants. This is because we replace 16 teeth with only four implants.
As with any method, there are contraindications and risks:
- serious liver and kidney diseases;
- uncompensated autoimmune conditions;
- some blood disorders;
- endocrine disorders.
How does it work?
Preparation for surgery

All-on-4 implants provide for detailed planning of the surgery. We will carry out a detailed diagnosis, including a computer tomography, scanning of the jaws, and calculation of future loads. Preparation includes making templates and temporaries for each step of treatment.
Implantation

On the day of surgery, we will place all implants. If indicated, we will extract certain teeth and restore soft tissues beforehand. We will place implants and a temporary denture. The duration of the surgery is 1-3 hours.
Prosthodontics

After tissue healing, which takes about one week, we will place a temporary denture for long-term use. A dental bar is used for strength. Crowns are usually made of metal-reinforced plastic and the artificial gum is made of acrylic. The temporary denture can be worn from 4 months to a year. Then it must be replaced by a permanent one, which is made from stronger materials that are adapted to the situation.

Important
If there is sufficient bone tissue volume, the dentist may suggest the option of All-on-6. This is because an orthopedic construction supported by a greater number of implants provides greater reliability and long-term use.
